Our clients consist exclusively of home health and hospice agencies, and we are looking for a clinician who understands Medicare requirements, OASIS documentation, PTA supervision, and complex mobility needs. The ideal candidate will have experience performing home health evaluations, supervising Physical Therapy Assistants (PTAs), completing Medicare-required documentation, and conducting seating and mobility assessments, including evaluations for power wheelchairs and other complex rehabilitation technology. Minimum Qualifications Licensed Physical Therapist in good standing. Minimum 2 years of experience as a Physical Therapist in home health and/or hospice . Experience working with Medicare-certified home health agencies. Strong knowledge of OASIS assessments and Medicare documentation requirements. Experience supervising and signing off on PTA documentation and treatment plans. Experience performing seating and mobility evaluations. Experience evaluating patients for power wheelchairs, custom wheelchairs, and other mobility equipment. Excellent clinical assessment, communication, and documentation skills. Essential Duties and Responsibilities Complete Start of Care (SOC) evaluations and assessments. Perform routine patient visits and ongoing therapy treatments. Complete Recertifications, Re-evaluations, Resumptions of Care (ROC), and Discharge Assessments . Develop and update individualized plans of care. Supervise Physical Therapy Assistants (PTAs) and provide clinical oversight. Review and sign off on PTA visits, progress notes, and treatment documentation. Complete OASIS documentation accurately and timely. Coordinate care with physicians, nurses, case managers, and home health agency staff. Preferred Qualifications Established relationships within the home health and hospice industry. Experience with multiple home health EMR platforms.
