You will assess individual needs, develop personalized treatment plans, and deliver expert physiotherapy interventions across various settings including, inpatient units, and home health environments. Your expertise will help patients regain independence and achieve their rehabilitation goals through compassionate care and evidence-based practices. Duties Conduct comprehensive patient evaluations to assess physical function, mobility, and pain levels using current clinical standards. Develop and implement individualized treatment plans that incorporate therapeutic exercises, manual therapy techniques such as trigger point therapy, and modalities tailored to each patient's condition. Manage discharge planning processes to ensure seamless transition of care from hospital or inpatient settings to home or community environments. Collaborate closely with healthcare professionals including physicians, nurses, occupational therapists, and social workers to coordinate patient care effectively. Maintain accurate and detailed clinical documentation adhering to medical terminology standards and electronic health records (EHR) management protocols. Provide patient education on injury prevention, self-care techniques, and home exercise programs to promote ongoing recovery outside clinical settings. Support patients with diverse needs such as geriatrics, pediatrics, neurology, orthopedics, and outpatient rehabilitation by applying specialized physiotherapy techniques. Requirements Valid licensure or certification as a licensed Physical Therapist in the applicable jurisdiction. Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, inpatient units or home health environments. Strong knowledge of anatomy, physiology, medical terminology, and clinical documentation standards. Demonstrated ability to develop effective treatment plans for patients with orthopedic, neurological or geriatric conditions. Experience working with electronic health records (EHR) or electronic medical records (EMR) systems for documentation and patient tracking. Excellent client communication skills with a compassionate approach to patient care and education. Prior experience in discharge planning, working with geriatric patients or pediatric populations is highly desirable.
