You will work closely with nursing staff, physicians, families, and the rehabilitation team to help patients achieve their recovery goals. Essential Responsibilities Evaluate residents and develop individualized treatment plans Provide skilled physical therapy services in accordance with physician orders and established care plans Improve patient mobility, balance, strength, coordination, and functional abilities Accurately document evaluations, treatment progress, and patient outcomes Collaborate with physicians, nursing staff, and interdisciplinary team members Educate patients and families regarding treatment plans, exercises, and safety techniques Monitor patient progress and adjust treatment programs as needed Ensure compliance with state regulations, facility policies, and professional standards of practice Participate in care planning and discharge planning activities Qualifications Required Current and unrestricted Physical Therapist (PT) License Graduate of an accredited Physical Therapy program Strong clinical assessment and treatment skills Excellent communication and interpersonal abilities Ability to work effectively within a multidisciplinary healthcare team Preferred Experience in a Skilled Nursing Facility (SNF), Post-Acute Care, Rehabilitation, Long-Term Care, or Acute Care setting Knowledge of Medicare, therapy documentation, and rehabilitation best practices Ideal Candidate Patient-focused and compassionate Strong critical thinking and problem-solving skills Positive, team-oriented attitude Committed to achieving outstanding patient outcomes Passionate about restoring independence and improving quality of life Apply Today If you're looking for a rewarding career where you can make a lasting impact on patients' recovery journeys, we'd love to hear from you.
