Overview We are seeking qualified physical therapists to join our team to work in the assigned SNF/LTC facility. Duties Assess patients' physical abilities and limitations through comprehensive evaluations. Develop personalized treatment plans based on patient needs and goals. Implement therapeutic exercises and modalities to improve mobility and strength. Educate patients and their families on rehabilitation processes, home exercises, and preventative measures. Collaborate with other healthcare professionals to ensure comprehensive patient care. Document patient progress accurately in compliance with healthcare regulations. Provide support in acute care settings as needed, ensuring the highest level of patient safety and comfort. Stay updated on advancements in physical therapy practices and techniques. Skills Strong knowledge of anatomy and physiology relevant to physical therapy. Experience with geriatrics and SNF, a plus Excellent communication skills for effective interaction with patients, families, and interdisciplinary teams. Ability to develop effective treatment plans tailored to individual patient needs. Proficient in using physical therapy equipment and technology. Strong problem-solving skills to address patient challenges effectively. Compassionate approach towards patient care with a focus on improving quality of life.
