Overview Partners CA Home Health is seeking a dedicated and experienced Physical Therapist (PT) - Per Diem to provide skilled physical therapy services to patients in their homes throughout our service area. The Physical Therapist will evaluate patients, develop individualized treatment plans, provide therapeutic interventions, and help patients improve mobility, strength, balance, safety, and overall independence. This position is ideal for a clinician who is comfortable working independently in the home health setting while communicating closely with physicians, nurses, occupational therapists, speech therapists, and other members of the care team. This is a flexible per diem position with visits assigned based on patient need and therapist availability. Responsibilities Perform comprehensive physical therapy evaluations in the patient's home. Assess mobility, strength, balance, endurance, gait, range of motion, pain, and functional limitations. Develop individualized treatment plans based on physician orders, patient needs, and established goals. Provide skilled physical therapy treatments to improve mobility, strength, balance, safety, and functional independence. Provide gait training, transfer training, therapeutic exercise, balance training, and other appropriate physical therapy interventions. Educate patients and family members on home exercise programs, fall prevention, safety, proper use of assistive devices, and techniques to improve independence. Monitor patient progress and update treatment plans as needed. Complete reassessments and discharge evaluations when appropriate. Communicate changes in patient condition to the physician and other members of the interdisciplinary care team. Coordinate care with nurses, occupational therapists, speech therapists, home health aides, and other healthcare professionals. Complete all required documentation accurately and within agency timeframes. Maintain compliance with Partners CA Home Health policies, Medicare requirements, payer requirements, and professional standards of practice. Participate in care coordination and case communication as needed. Qualifications Current and unrestricted California Physical Therapist license . Graduate of an accredited Physical Therapy program. Home health experience preferred. Experience with geriatric, orthopedic, neurological, and medically complex patients is preferred. Strong assessment, treatment planning, and clinical documentation skills. Ability to work independently and manage a flexible patient schedule. Comfortable using electronic medical record systems. Strong communication and interpersonal skills. Reliable transportation and ability to travel to patient homes within the assigned service area. Current CPR/BLS certification, if required by agency policy. Must meet all applicable Partners CA Home Health employment and credentialing requirements. Schedule Per Diem / As Needed Patient visits are assigned based on agency census, geographic location, patient needs, and therapist availability. No minimum number of visits is guaranteed. Compensation Per-visit compensation based on the Partners CA Home Health current per diem pay schedule.
