In this role, you will design and implement evidence-based care plans, promote community integration, and work closely with family members and clinical staff to support successful discharge planning.

Key Responsibilities Clinical Care & Assessment:

Perform comprehensive examinations (history, systems review, tests/measures) and evaluate client mobility status to diagnose movement dysfunction.

Treatment Planning:

Develop and execute individualized intervention strategies addressing transfers, bed mobility, gait training, and community mobility.

Fitness & Wellness:

Provide practical training in individual fitness programs, cardiovascular testing, and the proper use of exercise equipment.

Community Integration:

Deliver direct and consultative services to promote higher-order community integration and recommend adaptive equipment.

Collaborative Discharge Planning:

Teach strategies to team members, train families/caregivers, and assist with securing setting-specific discharge resources.

Documentation:

Complete evaluations, monthly progress reports, and therapy session notes within established company and regulatory timeframes.

Compliance:

Maintain strict adherence to APTA Code of Ethical Practice, State Practice Acts, DDD, HIPAA, and CARF guidelines. Schedule & Hours Minimum commitment: 15-20 hours per week Hours must be worked during the days and hours of Monday-Friday 8:30am-5:00pm This is a home and community-based position. Travel may account for up to 30% of working time.

Qualifications Education:

Bachelor's degree in Physical Therapy required; Master's degree or professional certification is preferred.

Licensure:

Valid CA State Physical Therapist License is required.

Experience:

Experience treating neurologically related physical disorders in a community-based setting is strongly preferred.

Skills:

Excellent time management, organizational, and writing skills to handle documentation alongside direct patient care.

Transportation:
