Manage discharge planning processes to ensure seamless transition from acute care or inpatient settings to home or community-based care. Document all patient interactions accurately in electronic health records (EHR) systems while adhering to clinical documentation standards. Provide patient education on treatment procedures, injury prevention, and self-care techniques to promote long-term wellness. Assist with treatment plan development for diverse populations including geriatrics, pediatrics, neurology patients, and those requiring outpatient or home health services. Skills Strong knowledge of physiology, anatomy, and medical terminology relevant to physical therapy practice. Proven experience working with geriatric patients in outpatient clinics or home health environments. Skilled in outpatient rehabilitation center procedures and inpatient hospital settings with a focus on patient care excellence. Ability to develop effective treatment plans tailored to individual needs across various specialties such as orthopedics and neurology. Proficiency in clinical documentation standards and electronic health records (EHR/EMR systems) management. Excellent client communication skills to educate patients about their treatment process and foster motivation for recovery. Experience with discharge planning, care plan creation, and working within multidisciplinary teams for comprehensive patient management.
