HOME HEALTH

licensed Physical therapist, you will be responsible for evaluating and treating patients with various physical conditions to improve their mobility and quality of life. You will work in a home environment, collaborating with other healthcare professionals to develop effective care plans tailored to each patient's needs. Your expertise in medical terminology and geriatrics.

We also offer Mentorship from a very experienced clinical team. Duties Conduct thorough assessments of patients' physical abilities and limitations. Develop individualized care plans that address the specific needs of each patient. Implement physical therapy interventions including exercises, manual therapy, and modalities. Monitor patient progress and adjust treatment plans as necessary. Educate patients and their families on treatment plans, exercises, and self-care strategies. Collaborate with other healthcare professionals to ensure comprehensive patient care. Document patient evaluations, treatments, progress notes, and discharge planning accurately in compliance with home care documentation systems. Stay updated on advancements in physical therapy practices and medical terminology. Qualifications Doctor of Physical Therapy (DPT) degree from an accredited program. Valid California state license to practice physical therapy. Willing to begin right away or in the near future. Open to starting this position per Diem and then transitioning to part time/full diem. New Grads welcome to apply. Will provide Mentorship to get adjusted in the home health field. 1099

Valid credentials:

CPR or BLS within the last 2 years Valid California state Physical Therapist license. Driver's license Valid Auto insurance Tuberculosis (TB) test within the last 1 year Professional liability insurance Business License from your local city
