$80-$160 Job Summary Join our dynamic healthcare team as a Home Health Physical Therapist where you will play a vital role in delivering exceptional patient-centered care within the comfort of patients' homes. This position offers an exciting opportunity to apply your expertise in physical therapy, foster meaningful patient relationships, and collaborate with a multidisciplinary team to promote recovery, mobility, and independence. You will be responsible for implementing tailored treatment plans, conducting comprehensive assessments, and ensuring high-quality documentation to support optimal patient outcomes. Duties Develop and implement individualized care plans based on thorough clinical assessments and patient needs. Perform physical therapy treatments including manual therapy, trigger point therapy, and therapeutic exercises to improve mobility and reduce pain. Conduct detailed clinical documentation adhering to standards for medical documentation and electronic health records (EHR) management: Kinnser/Wellsky Collaborate effectively with healthcare professionals such as physicians, nurses, and occupational therapists to coordinate patient care strategies. Educate patients and caregivers on treatment techniques, safety measures, and discharge planning to ensure continuity of care. Utilize medical equipment operation skills safely and efficiently during therapy sessions. Maintain compliance with clinical assessment documentation standards, medical terminology accuracy, and physiotherapy best practices.
Qualifications:
Minimum of 1 year of home health experience Must have a current PT License in California Valid California Driver's License, reliable transportation and auto insurance. Knowledge with Kinnser/Wellsky EMR is a plus; but not required. This role is paid and offers a rewarding environment where your skills directly impact patients' quality of life.
