Minimum 1 Year Required About the Role We are seeking an experienced Physical Therapist (PT) Home Health for a 13-week contract position serving Fremont and surrounding East Bay communities, including Castro Valley, Hayward, Newark, San Leandro, San Lorenzo, Union City, Oakland, Berkeley, Alameda, Emeryville, and nearby areas. The Physical Therapist will provide home-based rehabilitation services throughout the assigned territory. Additional travel outside the primary service area and weekend coverage may be required to maintain weekly patient visit and productivity expectations. Responsibilities Evaluate patients in their homes and provide individualized physical therapy services based on clinical needs and functional goals. Complete Start of Care (SoC), evaluations, recertifications, resumptions of care, routine visits, reassessments, and discharge visits as assigned. Maintain timely and accurate clinical documentation for all patient visits and therapy services. Coordinate patient care and communicate with physicians, patients, caregivers, and other healthcare professionals. Travel throughout the assigned service territory and support additional areas or weekend visits as needed to meet caseload expectations. Requirements Active California Physical Therapist (PT) License Required. Minimum 1 Year of Physical Therapy Experience Required. Current BLS Certification Required. Valid Driver's License, Auto Insurance, and Vehicle Registration Required. Current TB Clearance and Required Health/Immunization Documentation Required. The Physical Therapist will provide patient-centered Home Health rehabilitation services focused on improving mobility, function, safety, and independence.
