• $165 an hour 7 hours ago Benefits Travel reimbursement Flexible schedule Qualifications Driver's License Physical Therapy License Full Job Description Full Job Description We are seeking a dedicated and skilled Home Health Physical Therapist (PT) to join our patient-centered healthcare team.

In this role, you will deliver high-quality, 1-on-1 rehabilitative care directly to patients in their homes or residential care settings. You will be responsible for evaluating physical functionality, developing individualized plans of care, and providing therapeutic interventions designed to restore mobility, prevent falls, and improve overall independence.

Key Responsibilities In-Home Evaluations:

Conduct physical therapy evaluations, assessing strength, range of motion, balance, gait, transfer safety, and home safety hazards.

Treatment Planning:

Develop and execute individualized care plans focused on restoring functional mobility, pain relief, and fall prevention.

Therapeutic Interventions:

Deliver hands-on therapy including manual techniques, exercise programs, gait retraining, trigger point therapy, and orthopedic rehabilitation.

Patient & Caregiver Education:

Train patients and family members on home exercise programs (HEP), safe transfer techniques, and proper use of durable medical equipment (DME).

EMR & Documentation:

Timely complete OASIS assessments, initial evaluations, progress notes, and discharge summaries in accordance with California regulations and company standards.

Interdisciplinary Care:

Collaborate with physicians, nurses, occupational therapists, speech therapists, and social workers to ensure seamless patient care and discharge planning.

Supervision:

Provide supervision to Physical Therapist Assistants (PTAs) in accordance with the Physical Therapy Board of California guidelines.

Qualifications & Requirements Education:

Doctorate or Master's degree in Physical Therapy from an accredited program.

Licensure:

Valid, unencumbered Physical Therapist license issued by the Physical Therapy Board of California (PTBC).

Certifications:

Current CPR/BLS certification for Healthcare Providers.

Field Requirements:

Valid California Driver's License Reliable personal vehicle with active auto insurance Ability to pass required California background checks and health screenings (TB test, immunizations)

Experience:

Minimum 1 year of physical therapy experience preferred; experience in Home Health, OASIS documentation, geriatrics, or acute care is a strong plus.

Skills:

Excellent clinical judgment, effective communication, strong organizational skills, and comfort using mobile EMR applications. Please note this is a per diem position ($140
