Grace Community Home Health is a Christian Faith-Based Agency seeking a per diem Physical Therapist to provide excellent care as part of a team who loves to serve our community. Physical Therapist Home Health Position Summary Full independence and schedule flexibility Independently evaluate and conduct follow-up visits as a Physical Therapist Provide Physical Therapy services to clients in their home Document patient assessment and visit details in EHR and notify Clinical Supervisor of findings Communicate care and updates with the care team Submit documentation of Evaluation and Follow up visits according to the company policies Evaluate and create plan of care for patients independently Qualifications Friendly, cheerful, and excellent customer service/bedside manners Minimum of 1-year experience seeing patients in the home setting (preferred) 2 years as a Physical Therapist in an acute or rehab facility (preferred)

Active Physical Therapy California License Pay:
