In this role, you will deliver personalized treatment plans, promote recovery, and foster independence for diverse patient populations, including geriatric, pediatric, neurological, and orthopedic cases. Your proactive approach and clinical skills will ensure seamless coordination with healthcare professionals to optimize patient outcomes. Responsibilities Assess patients' physical conditions and develop tailored treatment plans to improve mobility, strength, and function within the home environment Deliver hands-on therapy sessions including therapeutic exercises, manual therapy, trigger point therapy, and other modalities aligned with best practices in physiotherapy Conduct discharge planning to ensure smooth transitions from hospital or outpatient settings to home care Collaborate closely with physicians, nurses, caregivers, and other healthcare providers to coordinate patient care and update treatment strategies Maintain accurate and comprehensive clinical documentation adhering to standards for medical terminology and electronic health records (EHR) management Educate patients and their families on exercises, safety precautions, and self-care techniques to promote ongoing recovery and prevent re-injury Monitor patient progress regularly, adjusting treatment plans as needed based on clinical assessments and physiology knowledge Qualifications Valid license or certification as a licensed Physical Therapist practicing in a home health setting Proven experience in outpatient rehabilitation centers, inpatient hospital units, or outpatient clinics with exposure to geriatrics, orthopedics, neurology, pediatrics, or home & community care environments Strong understanding of anatomy, physiology, medical terminology, and clinical documentation standards Demonstrated ability to develop effective treatment plans tailored to individual needs across diverse patient populations Experience working with electronic health records (EHR) or electronic medical records (EMR) systems for documentation and care coordination Excellent client communication skills with the ability to motivate and educate patients of all ages Ability to work independently while collaborating effectively with healthcare teams in fast-paced settings This role is an excellent opportunity for passionate physical therapists eager to apply their skills in a rewarding home health environment.
