$120.00•140.00 per completed patient visit Job Summary Care Bridge Group Inc. is seeking an experienced, compassionate, and dependable Physical Therapist (PT) to provide skilled home health services to patients in their homes. We partner with Medicare-certified home health agencies throughout Southern California and are looking for clinicians who are committed to delivering high-quality, patient-centered care. This position offers flexible scheduling, competitive per-visit compensation, and the opportunity to work independently while making a meaningful impact in patients' lives. Responsibilities Perform comprehensive physical therapy evaluations. Develop individualized treatment plans based on physician orders and patient needs. Provide therapeutic exercises, gait training, balance training, transfer training, strengthening, mobility training, and fall prevention education. Assess patient functional status and progress toward established goals. Educate patients and caregivers regarding home exercise programs and safety techniques. Recommend appropriate durable medical equipment (DME) and assistive devices. Communicate patient progress with physicians, nurses, case managers, and other interdisciplinary team members. Complete reassessments and discharge evaluations as appropriate. Maintain accurate, timely, and complete clinical documentation. Submit all visit documentation within 48 hours of each patient visit. Comply with Medicare Conditions of Participation, HIPAA, infection control standards, and agency policies. Qualifications Current California Physical Therapist License in good standing. Graduate of an accredited Physical Therapy program. Current CPR/BLS Certification. Minimum one (1) year of Home Health experience preferred. Excellent assessment and documentation skills. Valid California driver's license, reliable transportation, and current automobile insurance. Ability to travel within assigned service areas.
