Your Role:

Home Health Physical Therapist (PT) As a Physical Therapist in Home Health , you will provide skilled rehab services tailored to each patient's needs. Your focus will include fall prevention, strength building, mobility training, and functional independence for senior and geriatric patients .

Key Responsibilities:

Conduct in-home physical therapy evaluations and reassessments Develop and implement personalized rehabilitation care plans Improve strength, balance, mobility, and functional outcomes Provide therapy in home health, assisted living, and community care settings Educate patients and families on fall prevention and safe mobility techniques Document visits using WellSky (Kinnser) EMR Collaborate with interdisciplinary home health teams Qualifications Licensed Physical Therapist (PT)
