Job Description

Job Summary Join our dynamic team as a Full-Time Physical Therapist dedicated to making a meaningful difference in the lives of our diverse patient population through our innovative PACE (Program of All-Inclusive Care for the Elderly) program. In this vital role, you will deliver comprehensive physiotherapy services, develop personalized care plans, and collaborate closely with interdisciplinary teams to promote optimal mobility, independence, and quality of life. Your expertise will empower seniors and individuals with complex health needs to thrive in their community settings while ensuring safe and effective patient care. Duties Assess patients' physical conditions, including mobility, strength, and functional abilities, to inform tailored treatment strategies. Design and implement individualized care plans that incorporate physiotherapy techniques such as trigger point therapy, orthopedics, and geriatrics-focused interventions. Provide hands-on therapy sessions in various settings including inpatient hospital units, outpatient clinics, home health visits, and community centers. Coordinate discharge planning by evaluating patients' progress and collaborating with healthcare teams to ensure seamless transitions across care settings. Educate patients and caregivers on exercises, activity modifications, and injury prevention strategies to promote ongoing health and wellness. Maintain accurate documentation within electronic medical record (EMR) systems, including progress notes, treatment updates, and discharge summaries. Stay current with medical terminology, anatomy knowledge, physiology principles, and emerging physiotherapy practices to deliver evidence-based care. Experience Proven experience in acute care hospital environments, inpatient units, outpatient clinics, or home health settings demonstrating strong patient care skills. Familiarity with discharge planning processes and developing comprehensive care plans for diverse populations including pediatrics and geriatrics. Hands-on experience with physiotherapy techniques such as trigger point therapy and orthopedic rehabilitation. Knowledge of medical terminology, anatomy, physiology, and the application of these concepts in clinical practice. Prior work in home & community care environments or outpatient clinics preferred; experience with EMR systems is essential. $62.00 - $68.00 per hour
