Job Description

All Care and Comcare Home Health is seeking a compassionate and motivated Physical Therapist to join our growing team. This role allows you to provide high-quality, individualized care in the comfort of patients' homes while enjoying a f lexible schedule and supportive team environment .
Position:
Field Physical Therapist Key Responsibilities:
Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders. Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC). Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals. Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals. Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.
Qualifications:
Active license/registration with the Physical Therapy Board of California. Graduate of an accredited Physical Therapy program. Strong documentation and organizational skills. Excellent communication and interpersonal abilities. Willingness to travel to patient homes for evaluations and treatment. Flexibility regarding work locations throughout our service area.

Requirements:
Valid California Physical Therapy license. Current CPR certification. Valid California driver's license. Current California auto insurance. Completed physical examination, TB test, and required vaccines/titers. Two professional references.
