Your clinical skills and compassionate approach will help develop personalized treatment plans, facilitate patient rehabilitation, and collaborate seamlessly with healthcare professionals to ensure optimal recovery outcomes. Duties Conduct comprehensive assessments of patients' physical conditions, including evaluating physiology, anatomy, and neurological status. Develop and implement individualized care plans tailored to each patient's needs, including discharge planning and treatment strategies. Deliver evidence-based therapies such as orthopedics, neurology, pediatrics, geriatrics, trigger point therapy, and home & community care. Document patient progress accurately in electronic health records (EHR) systems while adhering to clinical documentation standards. Collaborate with physicians, nurses, and other healthcare team members to coordinate care and adjust treatment plans as necessary. Educate patients and their families on physiotherapy techniques, mobility exercises, and injury prevention strategies. Manage outpatient rehabilitation sessions at clinics or inpatient units, ensuring safety and adherence to best practices in patient care. Experience Prior experience practicing in a physical therapy role within outpatient clinics or inpatient hospital settings is highly preferred. Demonstrated proficiency in discharge planning, treatment plan development, and clinical documentation standards. Hands-on experience working with geriatric patients, pediatric populations, or neurological cases is advantageous. Knowledge of medical terminology related to orthopedics, neurology, anatomy, and physiology is essential for accurate assessment and documentation. Familiarity with electronic health records (EHR) or electronic medical records (EMR) systems for managing patient data efficiently. Experience in home health or outpatient rehabilitation centers enhances your ability to deliver comprehensive patient care across various environments.
