Treat Patients Where Daily Life Happens A clinic can show you how someone performs in a controlled environment. Home-based care shows you how that person actually functions. By treating patients in their homes, you can evaluate the real barriers affecting their safety and independence, including: Stairs and entryways Bathrooms and transfers Flooring and fall hazards Household mobility Daily routines Caregiver needs Community access This allows you to build treatment plans around the patient's actual environment, priorities, and functional goals. Provide Focused, One-on-One Care You will spend one full hour with each patient , allowing you to evaluate thoroughly, observe movement, provide individualized treatment, educate patients and caregivers, and make thoughtful clinical decisions. With approximately five patients per day on average, you will have time to deliver skilled care without moving through a high-volume clinic schedule. Have Autonomy Over Your Day You will be trusted to manage your patients according to their plans of care, organize your schedule, communicate with referring providers, and make appropriate clinical decisions. You will have the independence to practice as a professional while still receiving mentorship, collaboration, and support when you need it. Help Build New Programs ReNeu is a growing private practice where strong ideas can become real programs. If you are interested in helping develop balance classes, Parkinson's programs, caregiver education, community workshops, or other neurologic health initiatives, you will have the opportunity to contribute. You may also be supported in building your professional presence through community presentations, educational events, workshops, and provider in-services. What You Will Do as a Physical Therapist Perform comprehensive Physical Therapy evaluations in patients' homes Assess mobility, balance, gait, strength, coordination, endurance, safety, and functional independence Develop individualized plans of care based on each patient's condition, environment, needs, and goals Provide skilled, one-on-one treatment during 60-minute appointments Help patients improve mobility, balance, movement quality, confidence, and participation in everyday activities Provide treatment for individuals with Parkinson's disease and other neurologic conditions Address fall risk and recommend practical strategies to improve safety in the home Educate patients and caregivers on exercises, movement strategies, safety, and long-term condition management Schedule patients according to their established plans of care Monitor progress and adjust treatment plans based on clinical findings and patient response Communicate with referring physicians and other healthcare providers to coordinate care Complete accurate and timely clinical documentation Travel to patients throughout the Denver and west metro service area Contribute ideas that support practice growth, patient education, and future clinical programs What We Are Looking For Doctor of Physical Therapy degree or equivalent Physical Therapy degree Current or eligible Colorado Physical Therapist license Approximately one to two years of Physical Therapy experience preferred Outpatient neurologic rehabilitation experience strongly preferred Experience working with individuals with Parkinson's disease preferred PWR! Moves certification preferred Strong clinical reasoning and independent decision-making skills Confidence managing a caseload and coordinating patient schedules Strong communication and patient education skills Ability to build trusted relationships with patients, caregivers, physicians, and other healthcare professionals Interest in neurologic rehabilitation, balance training, mobility, and fall prevention Comfort traveling between patient homes throughout the service area Reliable transportation Organized, dependable, and comfortable working independently New graduate Physical Therapists with a strong interest in neurologic care may be considered.
