Minimum 2 Years Required About the Role We are seeking an experienced Physical Therapist (PT) Outpatient for a 24-week contract in Glenwood Springs, Colorado. The Physical Therapist will evaluate patients and develop individualized treatment plans for a variety of diagnoses in a full-time outpatient rehabilitation setting. The therapist will provide interventions focused on improving strength, range of motion, balance, safety, mobility, and functional independence while educating patients and caregivers on appropriate home exercise programs. Responsibilities Evaluate patients to identify functional limitations, mobility deficits, and individualized rehabilitation goals. Develop and implement patient-specific physical therapy treatment plans based on clinical findings and diagnoses. Provide therapeutic interventions designed to improve strength, range of motion, balance, safety, and functional independence. Monitor patient progress and modify treatment plans based on clinical response and rehabilitation outcomes. Educate patients and caregivers on home exercise programs while maintaining accurate clinical documentation and coordinating care with the rehabilitation team. Requirements Minimum 2 Years of Physical Therapist Experience Required; 3 5 Years of Experience Preferred. Current Colorado Physical Therapist License Through DORA or Eligible Colorado Physical Therapy Compact Privilege Required. Current American Heart Association BLS Certification Required. Bachelor's Degree in Physical Therapy Required. Flu Vaccination Required From October 1 Through April 1 With No Declinations Accepted; Permanent Residence Must Be at Least 75 Miles From the Assignment Location.
