Job Description

Physical Therapist (PT) — Moab, Utah | PRN |
Home Health Schedule:
We'll work with your schedule to find days and times that work for you and our patients. Position Summary CNS is seeking a compassionate PRN Physical Therapist to support Home Health patients in the greater Moab, Utah area. This role offers meaningful 1:1 care, greater autonomy, and the opportunity to work with patients needing care in the home. Compensation & Benefits Hourly /
Per Visit:
Starting at $65.00 to $95.00 per visit — based on experience, education, and work history. The hourly rate is per visit; Indeed doesn't give a per-visit option.
Stipend:
We will also pay you a travel stipend for driving from Grand Junction to Moab. Talk to the recruiter for details.
Mileage:
$0.57/mile reimbursement — when in town on visits. Why Home Care at CNS? Better work-life balance than facility-based shifts and patient ratios 1:1 patient care with strong clinical support Autonomy with fewer interruptions Meaningful relationships built in the home environment Key Responsibilities Provide skilled physical therapy services to Home Health patients in their homes Conduct comprehensive patient evaluations and develop individualized treatment plans Implement therapeutic exercises, manual therapy, gait training, and functional mobility interventions Educate patients, families, and caregivers on home exercise programs and safety modifications Coordinate care with physicians, nurses, occupational therapists, social workers, and interdisciplinary team members Monitor patient progress and adjust treatment plans as needed Maintain accurate and timely documentation per CMS, state, and CNS standards Adhere to safety, infection control, and HIPAA guidelines Qualifications Current Utah PT license in good standing Graduate of an accredited physical therapy program (DPT preferred) CPR/BLS certification required Home health experience preferred; pediatric, geriatric, neurological, or orthopedic experience a plus Strong clinical assessment, critical thinking, and problem-solving skills Excellent communication and interpersonal skills Ability to work independently and manage time effectively with a field-based caseload Valid driver's license, reliable transportation, and current auto insurance Physical Requirements Ability to lift, transfer, and assist patients with mobility (up to 50 lbs frequently) Ability to drive throughout the Moab area and surrounding communities Ability to sit, stand, walk, bend, kneel, and squat for extended periods Ability to carry therapy equipment and supplies to patient homes This job summary outlines the general nature and level of responsibilities and is not an exhaustive list of all duties or skills required.
