1 year of Medicare certified Home Health experience. 1 year of HCHB EMR and OASIS experience.

Responsibilities:

Provide one-on-one physical therapy services to patients in their homes. Complete patient assessments and document progress toward therapy goals. Develop and adjust individualized care plans based on patient needs and response to treatment. Coordinate care with families and other healthcare team members to support patient outcomes.
