  • Outpatient Rehab Lead with
Expertise:
Upon referral, conduct comprehensive evaluations of patients' physical function—including strength, mobility, motor control, joint health, pain levels, balance, posture, and cardiopulmonary performance. You'll assess vitals, endurance, gait, and daily living capabilities with precision and compassion.
Create Impactful Care Plans:
Design personalized, patient-centered treatment plans that promote optimal movement, safety, and long-term wellness. Your clinical insight will guide each step of the recovery journey.
Deliver Innovative Therapy:
Implement and delegate care using a wide range of therapeutic interventions—from heat, cold, and light therapy to mobilization and targeted exercise. You'll use your clinical judgment to tailor treatments that truly work. Collaborate for
Continuity:
Work closely with a multidisciplinary team—including Rehab Therapists, Nurses, and Social Workers—to ensure seamless care coordination. You'll also play a key role in discharge planning and connecting patients with community resources to support their ongoing recovery. About our Bloomfield Outpatient Rehab Clinic Join our thriving Bloomfield clinic , conveniently located in the Greater Hartford area and known for delivering exceptional, patient-centered outpatient rehabilitation services. Our collaborative team is dedicated to helping patients achieve their functional goals through evidence-based care in a supportive and welcoming environment. At the Bloomfield location, you'll have the opportunity to work with a diverse patient population and develop expertise across multiple specialty areas, including: Orthopedic Rehabilitation
  • Treat a wide range of musculoskeletal conditions, post-surgical patients, sports-related injuries, and joint replacement recoveries. Vestibular & Balance Rehabilitation
  • Help patients improve dizziness, vertigo, balance impairments, fall risk, and functional mobility. Lymphedema Management (Occupational Therapy Services)
  • Collaborate with rehabilitation specialists providing comprehensive lymphedema evaluation and treatment. Pelvic Floor Rehabilitation
  • Support patients experiencing pelvic health conditions through specialized, compassionate care designed to improve quality of life and function.
