The Physical Therapist is a qualified professional who directs, supervises, evaluates and provides direct care to patients and provides physical therapy services to patients in the home according State and Federal regulation under the direction of the attending physician. EEO/AA/Disability/Veteran Responsibilities Comprehensive evaluation of the patient's level of function and development of the therapy plan of care. Participation in case management conferences and documentation thereof. Instruction of patient, caregivers, PTAs, and other agency clinical staff in the patient's treatment plan including follow up supervision. Selects and administers diagnostic and therapeutic techniques and materials. Supervision of Physical Therapy Assistants. Confers with PTAs at least weekly and when there is a change in condition. Supervision of Home Health Aides when they are participating in the patient's therapy care plan. Documentation is completed in the EMR within 24 hours after visits are made. Assists patients and caregivers in obtaining necessary equipment and provides instruction on how to use equipment. Responsible for maintaining productivity and appropriately communicates needs for additional visits when necessary. Ensures that there is insurance authorization prior to making visits when appropriate. Reports incidents that occur to the RN case manager, and Manager, Therapy Services. Participates in staff meetings, in-service programs, and other meetings as requested. Qualifications
EDU CATION
Bachelor of Science in Physical Therapy required, masters or doctorate desirable.
EXPERIENCE
Minimum three years of clinical experience in Physical Therapy in an acute or sub-acute care setting. Home care experience desirable.
LICENSURE
Physical Therapist with Valid (or eligible) Connecticut license.
SPECIAL SKILLS
Excellent written and verbal communication. Ability to work collaboratively with multidiscipline. Must be adaptable, flexible, ability to learn quickly, use critical thinking skills, oversee PTAs. Strong computer skills.
PHYSICAL DEMAND
Ability to perform tasks involving physical activity which may include heavy lifting and extensive bending and standing.
Pay:
$42.97
