Job Description

Physical Therapist (PT) - Full-Time Civita Care Center at Long Ridge | Stamford, CT $45-$50/hour | Full-Time | Benefits Post Rehab is seeking a Physical Therapist (PT) to join the rehabilitation team at Civita Care Center at Long Ridge. This full-time opportunity allows you to work directly with residents to improve movement, restore strength, and build greater independence while collaborating with an experienced post-acute care team. What You'll Do Evaluate residents and develop individualized physical therapy plans of care Provide skilled treatment addressing mobility, gait, balance, strength, transfers, and endurance Establish functional rehabilitation goals and track resident progress Adjust treatment approaches based on resident needs and clinical response Educate residents and caregivers on exercises, mobility strategies, and safety techniques Maintain accurate and timely clinical documentation Partner with PTAs, OT, SLP, nursing, and other members of the interdisciplinary team What We Offer $45-$50/hour Full-time position Medical, dental, and vision insurance 401(k) Paid time off and holiday pay Continuing education support Collaborative rehabilitation team Short-term rehabilitation and long-term care caseload Opportunities for professional development Qualifications Active Connecticut Physical Therapist license required Graduate of an accredited Physical Therapy program Strong clinical, communication, and organizational skills Skilled nursing or rehabilitation experience preferred, but not required New graduates are welcome to apply If you're a PT looking for a collaborative environment where you can make a meaningful difference in residents' mobility and recovery, we'd love to hear from you.
