Physical Therapist (PT) - Home Health New Haven to Greenwich, CT | Contract | Home Health We are seeking a Physical Therapist (PT) to provide high-quality home health services to patients across the New Haven to Greenwich coverage area. Visits are scheduled in close geographic proximity to support efficient patient care and travel. Key Responsibilities Complete PT admissions and initial evaluations Perform resumption of care (ROC) visits and recertifications Provide routine physical therapy treatment visits Develop and implement individualized treatment plans Complete all clinical documentation within 24 hours of each visit Collaborate with physicians, caregivers, and interdisciplinary teams to ensure optimal patient outcomes Required Qualifications Doctorate Degree in Physical Therapy (DPT) Active Physical Therapist license Strong clinical assessment and patient care skills Home health experience preferred Preferred Qualifications Homecare Homebase (HCHB) experience OASIS documentation experience
