We are seeking a compassionate, patient-focused Physical Therapist (PT) to provide care primarily within our Washington, DC center, with occasional home visits throughout Northeast and Southeast Washington, DC. Assess participant mobility, strength, balance, gait, and functional status. Provide restorative, maintenance, and rehabilitative therapy services. Conduct initial evaluations, annual assessments, reassessments, and change-of-condition evaluations. Establish measurable goals and monitor participant progress toward improved function and independence. Collaborate with physicians, nurses, social workers, and other interdisciplinary team members. Participate in home safety evaluations and occasional participant home visits throughout NE and SE Washington, DC. Educate participants, family members, caregivers, and staff on mobility, safety, fall prevention, and functional independence. Recommend and assist with durable medical equipment needs. Maintain accurate documentation and care plans in compliance with regulatory standards. Required Qualifications Active Washington, DC Physical Therapist License (Required) Graduate of an accredited Physical Therapy program. Current driver's license and reliable transportation. Ability to travel for occasional home visits within Washington, DC. Strong assessment, treatment planning, rehabilitation, and documentation skills. Experience working with geriatric, rehabilitation, home health, skilled nursing, assisted living, outpatient therapy, or PACE populations preferred. Experience with fall prevention, functional mobility training, care planning, and senior wellness programs preferred. Proficiency with electronic medical records and Microsoft Office applications.
