Job Description

Take on a vital role providing high-quality care as a Home Health Physical Therapist in a dynamic, patient-centered environment. Enjoy the flexibility of independent fieldwork while positively impacting the functional independence and mobility of individuals in their homes.
Schedule & Contract Details:
Full-time, 13-week contract commitment Monday through Friday, 8:00 AM 5:00
PM Field-based:
travel to patient homes within an assigned territory (mileage reimbursement included) Caseload typically consists of 2532 points per week (approximately 57 visits daily, depending on visit types) Electronic documentation using Home Care Home Base (HCHB) Conduct Start of Care, Resumption, Recertification, and Discharge assessments with
OASIS Qualifications:
Active Physical Therapist (PT) license in the state of practice (in good standing) At least 1 year of recent home health physical therapy experience (field experience strongly preferred) Proficiency in OASIS assessments required Experience with Home Care Home Base (HCHB) preferred Reliable transportation, valid drivers license, and proof of auto insurance Demonstrated ability to work independently and manage a mobile, flexible schedule Strong clinical decision-making, functional assessment, and organizational skills
Responsibilities:
Complete comprehensive physical therapy evaluations and develop tailored treatment plans Deliver targeted interventions: therapeutic exercise, neuromuscular re-education, gait/transfer training, fall prevention, and pain management Track patient progress, adapt care plans, and communicate with interdisciplinary team members Educate patients and caregivers on exercises, safety, assistive devices, and home management strategies Maintain accurate and timely documentation in HCHB Participate in case conferences, interdisciplinary meetings, and ongoing learning as required Ensure compliance with regulatory and agency standards, including Medicare guidelines Enjoy weekly pay, medical/dental/vision insurance, 401(k) options, 24/7 support, mileage reimbursement, and opportunities for contract renewals.
