Job Description

At HCA Healthcare, we are committed to compassionate and competent therapeutic services, directly at our patients' homes. As a Home Health Physical Therapist, you will play a pivotal role in assessing the patient's physical conditions and formulating personalized treatment plans accordingly. You will collaborate with a multidisciplinary team to restore, maintain, and promote the optimal physical function of our patients. You will travel to patient's residences to provide rehabilitative services that are informed by each patient's stages of recovery. Responsibilities Perform initial and ongoing physical therapy assessments of patients' condition. Formulate treatment plans to achieve the established goals. Integrate input from patients and families into treatment plans. Implement therapeutic interventions tailored to each patient's needs. Educate patients and families on the treatment plan, expectations, and outcomes. Regularly reassess clinical signs and symptoms to determine treatment effectiveness. Keep comprehensive documentation of patient care and progress. Collaborate with other healthcare professionals to provide holistic care. Required Skills Strong assessment and diagnostic skills Proficiency in developing and implementing targeted treatment plans Ability to communicate complex information to patients and caregiving teams effectively Strong organizational and time management skills Capability to adapt treatments as patient needs change Knowledge of the use of physical therapy tools and technology
