Tender Touch Rehab Services seeks a compassionate Physical Therapist to provide evidence-based rehabilitation for seniors in skilled nursing and long-term care settings. You will perform comprehensive evaluations, design individualized treatment plans, and deliver hands-on interventions to maximize mobility, safety, and independence. Responsibilities Provide individualized physical therapy evaluations and treatment plans for seniors in skilled nursing and long-term care settings Deliver evidence-based therapeutic interventions to improve mobility, function, and independence Collaborate with an interdisciplinary care team to coordinate patient-centered treatment goals Document assessments, progress notes, and outcomes in accordance with regulatory and company standards Educate patients, families, and caregivers on home exercises, safety, and functional strategies Participate in clinical case conferences, quality initiatives, and fall-prevention or restorative programs Supervise and mentor PTA staff as appropriate and support professional development Ensure compliance with Medicare and other payer regulations and facility policies Contribute to a respectful, compassionate environment that upholds patient dignity Engage in continuing education to maintain clinical excellence and current best practices Required Skills Orthopedic and neuromuscular assessment Geriatric rehabilitation techniques Therapeutic exercise and manual therapy Balance, gait, and fall-prevention training Assistive device prescription and training Functional mobility and transfer training Care planning and interdisciplinary collaboration EMR documentation and clinical reporting Knowledge of Medicare and post-acute regulations Patient, family, and caregiver education
