In this vital role, you will deliver exceptional rehabilitative care to diverse patient populations, helping individuals regain mobility, reduce pain, and improve their quality of life. Your expertise in physiotherapy and anatomy will be essential in developing personalized treatment plans that foster recovery and independence. This position offers an exciting opportunity to work in a collaborative environment where patient care, clinical excellence, and professional growth are prioritized. Duties Assess patients' physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to determine appropriate treatment strategies. Develop and implement individualized care plans tailored to each patient's needs, including orthopedics, neurology, pediatrics, geriatrics, and home health cases. Conduct various therapeutic interventions such as trigger point therapy, patient rehabilitation exercises, and mobility training to promote recovery. Collaborate closely with healthcare professionals—physicians, nurses, occupational therapists—to coordinate care plans and ensure seamless patient management. Manage clinical documentation standards accurately within electronic health records (EHR) systems and electronic medical records (EMR), ensuring compliance with legal and organizational requirements. Engage in discharge planning by evaluating patient progress and preparing appropriate recommendations for continued care or home & community care services. Educate patients and their families on treatment procedures, injury prevention, and self-care techniques to empower ongoing recovery outside the clinical setting. Experience Proven experience practicing in a physical therapy role within outpatient clinics or outpatient rehabilitation centers. Demonstrated expertise working with geriatric patients, pediatrics, or neurological cases in outpatient or hospital environments. Strong background in outpatient experience with familiarity in outpatient clinic workflows and outpatient rehabilitation center operations. Knowledge of medical terminology relevant to physiotherapy, orthopedics, neurology, and geriatrics. Experience with inpatient or hospital settings is advantageous but not required; familiarity with inpatient care enhances versatility. Proficiency in managing outpatient treatment plans that include discharge planning and patient care documentation aligned with clinical standards. Ability to collaborate effectively with healthcare teams while maintaining excellent client communication skills for optimal patient outcomes.
