The ideal candidate will be responsible for assessing, planning, and implementing individualized treatment programs to improve patients' mobility and quality of life. This role requires a strong understanding of anatomy, physiology, and medical terminology, along with the ability to work effectively in various settings such as inpatient, outpatient, and home health care. Duties Conduct comprehensive evaluations of patients to determine their physical therapy needs. Develop and implement effective care plans tailored to each patient's specific conditions and goals. Provide therapeutic interventions including manual therapy, exercise prescription, and education on injury prevention. Collaborate with multidisciplinary teams to ensure coordinated patient care. Monitor patient progress and adjust treatment plans as necessary. Document patient assessments, treatments, and outcomes using EMR systems. Educate patients and their families on home exercises and self-management strategies. Participate in discharge planning to facilitate smooth transitions from therapy services. Skills Strong knowledge of anatomy and physiology relevant to physical therapy practices. Proficiency in medical terminology for effective communication within the healthcare team. Experience in various settings including acute care, outpatient clinics, geriatrics, pediatrics, and home health care. Familiarity with physiotherapy techniques such as trigger point therapy and orthopedics. Ability to create comprehensive care plans that address individual patient needs. Excellent patient care skills with a focus on empathy and support during recovery processes. Experience with discharge planning to ensure continuity of care post-treatment. Knowledge of EMR systems for accurate documentation and record keeping.
