Step into a rewarding role as a Home Health Physical Therapist, making a direct impact on patient lives by delivering skilled physical therapy in their homes. This full-time, 13-week contract offers a dynamic schedule and the autonomy of independent, field-based work while supporting recovery and functional independence for those in need.

Schedule:

Monday through Friday, 8:00 AM 5:00 PM Field-based in patients homes Caseload of approximately 2532 points per week (generally 57 visits per day, aligned with visit types)

Key Responsibilities:

Complete comprehensive in-home PT evaluations, including OASIS assessments when applicable. Develop, implement, and update individualized treatment plans to target mobility, strength, balance, gait, pain management, and patient safety.

Deliver skilled interventions:

therapeutic exercise, neuromuscular re-education, gait training, transfer training, and fall prevention. Educate patients and caregivers on exercises, safety protocols, aid use, and home therapy programs. Monitor patient progress, make necessary plan modifications, and update physicians and healthcare team members. Accurately document all care using Home Care Home Base (HCHB). Collaborate with nurses, OTs, SLPs, social workers, and home health aides during meetings and conferences. Uphold agency policies and Medicare home health regulations.

Qualifications:

Active PT license in good standing for the state of practice. Minimum 1 year of recent home health PT experience. Required experience with OASIS assessments. Proficiency with HCHB preferred. Strong clinical judgment and communication skills. Reliable transportation, current drivers license, and auto insurance. Ability to independently manage a field-based, flexible schedule.
