Job Description

Palmetto Subacute Care Center seeks a licensed Physical Therapist to provide compassionate, resident-centered rehabilitation in our skilled nursing and subacute facility. You will assess functional status, develop individualized treatment plans, and deliver evidence-based therapy to support recovery, mobility, and quality of life. Collaborating closely with nursing, physicians, and other therapists, you will document progress, educate residents and families, and contribute to a respectful, team-oriented culture focused on clinical excellence and meaningful patient outcomes. Responsibilities Evaluate residents' physical function and mobility and develop individualized treatment plans Provide therapeutic exercises, gait training, balance and strength programs to meet rehab goals Collaborate with nursing, therapy, and medical teams to coordinate resident-centered care Educate residents and families on home exercises, safety, and use of assistive devices Document assessments, treatment sessions, progress notes, and discharge plans accurately Monitor patient progress, adjust care plans, and communicate status to interdisciplinary team Ensure compliance with facility policies, clinical standards, and regulatory requirements Contribute to a compassionate, respectful environment that supports dignity and quality of life Required Skills Physical therapy evaluation Treatment planning Gait training Therapeutic exercise Balance and neuromuscular training Post-acute and subacute rehab Electronic medical record documentation Interdisciplinary care coordination Patient and family education Fall prevention and safety
