Coordinate discharge planning with multidisciplinary teams to ensure seamless transitions from hospital or outpatient settings to home care. Maintain accurate and thorough clinical documentation in compliance with healthcare standards and electronic health records (EHR) systems. Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals to optimize patient outcomes. Educate patients and their families on treatment procedures, injury prevention, and home exercise programs to promote ongoing recovery. Monitor patient progress regularly and adjust treatment plans accordingly to meet evolving needs. Qualifications Valid licensure as a licensed Physical Therapist in the State of Florida Experience in home health settings, outpatient clinics, inpatient hospitals, or rehabilitation centers preferred. Strong knowledge of physiology, anatomy, orthopedics, neurology, and geriatrics to inform effective treatment strategies. Familiarity with medical terminology and clinical documentation standards for accurate record-keeping. Experience working with diverse populations including pediatric patients and geriatric clients in community or home environments. Proficiency in EMR (Electronic Medical Records) or EHR management systems for documentation and communication purposes. Ability to develop comprehensive treatment plans aligned with patient goals and healthcare protocols.
