Job Description
Encompass Health seeks a pool Physical Therapist to deliver compassionate, evidence-based rehabilitation care in an inpatient setting. You will evaluate patients recovering from major illness or injury, develop individualized treatment plans, and provide therapeutic interventions to restore mobility, function, and independence. Collaborate closely with an interdisciplinary team, educate patients and families, and document in the EMR. Responsibilities Evaluate patients' functional status, mobility, and rehabilitation needs using standardized assessments Develop and implement individualized, evidence-based physical therapy treatment plans Provide therapeutic exercises, gait training, balance and functional mobility interventions Educate patients and families on home exercise programs, safety, and recovery expectations Collaborate with physicians, nurses, and the interdisciplinary rehab team on care plans Document assessments, treatments, and progress accurately in the EMRParticipate in discharge planning and make recommendations for continued services or equipment Ensure compliance with safety, regulatory, and professional practice standards Contribute to quality improvement and best-practice initiatives within the rehab team Required Skills Patient assessment Rehabilitation treatment planning Therapeutic exercise Gait and balance training Neuromuscular re-education Patient and family education Electronic medical records (EMR) documentation Interdisciplinary team collaboration Discharge planning Safety and fall-prevention protocols
