We are seeking a dedicated and compassionate Physical Therapist (PT) to provide high-quality home health services to patients in their homes. The Physical Therapist will evaluate patients, establish individualized treatment plans, and deliver skilled therapy services to improve mobility, strength, balance, and overall functional independence.Key Responsibilities:
Evaluate patients and develop individualized treatment plans Provide physical therapy services in accordance with the established Plan of Care Assess patient progress and modify treatment plans as appropriate Document evaluations, treatments, and patient outcomes accurately Educate patients and caregivers on therapeutic exercises, mobility techniques, and fall prevention Collaborate with physicians and the interdisciplinary care team Maintain compliance with all state, federal, Medicare, and agency regulationsQualifications:
Active Florida license as a Physical Therapist (PT) Minimum of 1 year of home health experience preferred Strong clinical assessment, documentation, and communication skills Ability to work independently and manage a flexible schedule Reliable transportation and a valid driver's licensePreferred Skills:
Experience with electronic medical records (EMR) Knowledge of Medicare guidelines and home health documentation standards Bilingual (English/Spanish) is a plusBenefits:
