Our clinical team is supported by experienced leadership, established processes, and technology designed to help clinicians provide efficient, patient-centered care. Position Responsibilities Provide high-quality, individualized therapy services to patients in their homes. Complete comprehensive assessments and develop appropriate treatment plans based on patient needs. Provide skilled therapy interventions in accordance with the patient's plan of care. Monitor patient progress and communicate changes to the appropriate members of the care team. Complete required clinical documentation accurately and timely. Collaborate with physicians, nurses, therapists, patients, and caregivers to promote positive outcomes. Educate patients and caregivers regarding treatment plans, safety, and home exercise programs. Maintain compliance with applicable federal, state, and agency requirements. Qualifications Current and unrestricted Florida license appropriate to the position.

PT/OT:

Graduate of an accredited physical or occupational therapy program.

PTA/OTA:

Graduate of an accredited assistant program. Home health experience is preferred but not required for all positions. Strong clinical, communication, organizational, and interpersonal skills. Ability to work independently and manage an assigned patient caseload. Reliable transportation and the ability to travel throughout the assigned service area.
