Duties Conduct comprehensive assessments of patients' physical function, mobility, and pain levels within their home environments. Develop and implement individualized care plans tailored to each patient's specific needs, goals, and medical history. Deliver evidence-based physiotherapy interventions such as therapeutic exercises, manual therapy, trigger point therapy, and gait training to facilitate recovery. Collaborate effectively with healthcare professionals including physicians, nurses, occupational therapists, and social workers to coordinate care and discharge planning. Maintain accurate and timely clinical documentation adhering to healthcare standards for electronic health records (EHR) systems and medical terminology. Educate patients and their families on treatment techniques, injury prevention strategies, and home exercise programs to promote ongoing health management. Manage discharge planning processes by assessing progress and adjusting treatment plans to ensure optimal outcomes for diverse patient populations across outpatient clinics, inpatient settings, or home health environments. Qualifications Valid license or certification as a Physical Therapist with practicing experience in outpatient rehabilitation centers or home health settings. Demonstrated expertise in physiotherapy modalities including orthopedics, neurology, pediatrics, geriatrics, and trigger point therapy. Strong knowledge of anatomy, physiology, medical terminology, and clinical documentation standards necessary for accurate patient care records. Prior experience working with geriatric patients or in community-based home health environments is highly preferred. Proven ability to develop effective treatment plans through assessment of patient needs and collaboration with multidisciplinary teams. Familiarity with electronic health records (EHR) or electronic medical record (EMR) systems for documentation management. Excellent client communication skills with a compassionate approach to patient care across diverse populations including inpatient hospital or outpatient clinic experience.
