Job Summary We are seeking a dynamic and compassionate Physical Therapist to join our healthcare team, dedicated to improving patient mobility, functionality, and quality of life. Duties Assess patients' physical conditions by reviewing medical histories, conducting physical examinations, and evaluating movement patterns. Develop comprehensive treatment plans tailored to individual patient needs, including goal setting and progress tracking. Implement therapeutic interventions such as manual therapy, trigger point therapy, exercise programs, and modalities to promote healing and mobility. Coordinate discharge planning and collaborate with healthcare professionals including physicians, nurses, and occupational therapists to ensure continuity of care. Maintain accurate clinical documentation adhering to standards for physiotherapy records, medical terminology, and electronic health records (EHR) systems. Educate patients and their families on treatment procedures, home exercises, injury prevention strategies, and community resources. Qualifications Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction. Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health environments. Strong knowledge of physiology, anatomy, neurorehabilitation techniques, orthopedics, and geriatrics Familiarity with clinical documentation standards and EMR (Electronic Medical Records) management systems such as EMR or EHR platforms. Demonstrated ability to develop effective treatment plans based on patient assessments and clinical data. Excellent client communication skills with the ability to motivate patients through care plans tailored for rehabilitation success.
