This is a home health position for an Physical Therapist (PT). The PT is responsible for following the Plan of Care and implementing interventions based on the POC and goals. PT will be responsible for maintaining contact with the supervisors, while coordinating care between patient and POC. The therapist will manage their own schedule and drive to patients throughout the day. This job is very autonomous, perfect for those who don't enjoy being kept working in a facility all day.
