Educate patients and caregivers on exercises, safety, and fall prevention.
Document progress and ensure compliance with Medicare Part B regulations.
Collaborate with the home health/ therapy team to coordinate care.
Adjust treatments based on patient progress and needs.
Experience in home health or outpatient therapy preferred.
Knowledge of Medicare Part B documentation and regulations.
Strong assessment, treatment planning, and communication skills.
Reliable transportation and valid driver's license.
Comfortable working independently and managing a flexible schedule.
This position requires Level 2 background screening through Florida's Care Provider Background Screening Clearinghouse.

We are seeking a Physical Therapist to provide in-home care for Medicare Part B patients. Help patients regain mobility, manage pain, and live independently-all from the comfort of their homes.Key Responsibilities:
