Job Description
Job Overview Join our dynamic healthcare team as a Physical Therapist, where you will play a vital role in restoring and enhancing clients' mobility and overall well-being. In this energetic and rewarding position, you will assess patient needs, develop personalized care plans, and implement evidence-based techniques to promote optimal function, safety and engagement in life. Our therapists work in Assisted Living Facilities utilizing the client's outpatient in home Part B benefits. There is no oasis/simplified notes, make your own schedule. Responsibilities Conduct comprehensive patient evaluations to determine physical therapy needs. Develop and implement tailored care plans that align with patients' goals and medical requirements, including discharge planning for seamless transitions. Collaborate with multidisciplinary teams Educate patients and their families on treatment procedures, post-discharge exercises, and falls and injury prevention strategies to promote long-term health. Maintain accurate documentation of patient progress within electronic medical record (EMR) systems while adhering to medical terminology standards. Monitor patient responses during therapy sessions and adjust treatment plans accordingly to optimize outcomes. Qualifications Valid licensure or certification as a licensed Physical Therapist Proven experience with geriatric population Demonstrated ability to develop comprehensive care plans tailored to client individual needs and interests Familiarity with EMR systems for efficient documentation and patient record management. Experience with discharge planning processes and collaboration with healthcare teams for seamless patient transitions. Excellent communication skills to effectively educate patients on treatment protocols and recovery strategies.
