CORA Physical Therapy is seeking a licensed Physical Therapist to provide evidence-based outpatient care in our Healthcare & Medical Services department. In this role, you'll evaluate, develop, and implement individualized treatment plans to restore function, mobility, and quality of life for diverse patient populations. Collaborate with a multidisciplinary team, educate patients and families, and document outcomes. Responsibilities Evaluate patients using standardized assessments to determine functional limitations and rehabilitation needs Develop and implement individualized, evidence-based treatment plans Provide hands-on therapeutic interventions, therapeutic exercise, and neuromuscular re-education Educate patients and families on home exercise programs, prevention, and self-management Collaborate with physicians, therapists, and support staff to coordinate care Accurately document evaluations, treatment sessions, progress notes, and discharge summaries in EMRTrack patient progress and adjust plans of care to achieve functional goals Ensure compliance with regulatory, ethical, and documentation standards Participate in quality improvement, clinical education, and mentorship activities Contribute to a patient-first, teamwork-driven clinic culture Required Skills Orthopedic physical therapy Neuromuscular re-education Therapeutic exercise prescription Manual therapy techniques Gait analysis and training Patient assessment and functional outcome measures Pain management modalities Treatment planning and progression Electronic medical records (EMR) documentation Patient and family education Outpatient rehab protocols Return-to-sport and work conditioning Evidence-based practice application Interdisciplinary team collaboration Time management and caseload prioritization
