Responsibilities Evaluate and treat residents in an assisted living community and or Home setting Develop and implement individualized treatment plans Collaborate with physicians, families, and the interdisciplinary care team Maintain accurate, timely documentation in the EMR Qualifications Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program Active Florida Physical Therapist (PT) license New graduates and experienced therapists are encouraged to apply
