Job Description
Fitness Quest Physical Therapy is seeking an Outpatient Physical Therapist to provide evidence-based rehabilitation, sports medicine, and wellness care to patients of all ages. In this role, you'll evaluate and treat a diverse caseload, design individualized treatment plans, and collaborate with a supportive, interdisciplinary team. Responsibilities Perform comprehensive patient evaluations, including musculoskeletal and functional movement assessments in an outpatient setting Develop and implement individualized, evidence-based plans of care to address pain, mobility, strength, balance, and functional goals Provide skilled physical therapy interventions, including therapeutic exercise, manual therapy, neuromuscular re-education, and appropriate modalities Educate patients and families on diagnosis, prognosis, home exercise programs, body mechanics, and strategies for long-term health and injury prevention Document evaluations, daily treatment notes, progress reports, and discharge summaries accurately and in a timely manner using EMR systems Collaborate with referring physicians and other healthcare providers to coordinate and optimize patient care Monitor patient progress, adjust treatment plans as needed, and establish clear, measurable goals and outcomes Maintain a safe, clean, and organized treatment environment that reflects the clinic's standards of care and professionalism Participate in team meetings, case discussions, and continuing education activities to support clinical excellence and professional growth Uphold all ethical, legal, and regulatory requirements, including state practice standards and payer documentation guidelines Required Skills Outpatient orthopedic assessment Neurologic and post-surgical rehab Therapeutic exercise prescription Manual therapy techniques Gait and balance training Functional movement analysis Patient evaluation and documentation EMR documentation systems Care plan development Pain management modalities Communication with physicians Patient and family education Outcome measurement and reporting Time management Interdisciplinary collaboration
