Coordinate discharge planning with multidisciplinary teams to ensure seamless transition from hospital to home or community care. Document patient progress accurately within electronic medical record (EMR) systems and adjust treatment strategies accordingly. Educate patients and their families on injury prevention, self-care techniques, and home exercise programs. Collaborate with healthcare professionals in acute care, geriatrics, pediatrics, sports injuries, and outpatient settings to optimize patient outcomes. Requirements Valid licensure as a Physical Therapist in the respective jurisdiction.
