We are seeking a Physical Therapist - Certified Lymphedema Therapist (CLT) evaluates, develops, and implements individualized treatment plans for patients in their homes, with specialized expertise in the evaluation and treatment of lymphedema and related disorders. The therapist provides evidence-based physical therapy interventions, Complete Decongestive Therapy (CDT), patient and caregiver education, and collaborates with physicians and the interdisciplinary team to improve mobility, reduce edema, prevent complications, and maximize patient independence while ensuring compliance with Medicare, Florida AHCA regulations, and agency policies. Essential Duties & ResponsibilitiesHome Health Physical Therapy Perform comprehensive physical therapy evaluations and reassessments. Develop individualized plans of care based on physician orders. Complete OASIS documentation for therapy-only cases as required. Perform evaluations, reassessments, recertifications, and discharge visits. Implement therapeutic exercise, gait training, balance training, strengthening, and mobility interventions. Assess patients for durable medical equipment and adaptive equipment needs. Develop and update individualized home exercise programs. Coordinate care with physicians, nurses, occupational therapists, speech therapists, social workers, and home health aides. Communicate significant changes in patient condition promptly. Complete all clinical documentation accurately and within required timeframes. Participate in interdisciplinary case conferences and quality improvement activities. Lymphedema Specialty Responsibilities Evaluate patients with primary and secondary lymphedema. Develop individualized lymphedema treatment plans using Complete Decongestive Therapy (CDT). Perform Manual Lymphatic Drainage (MLD) when clinically appropriate. Apply multi-layer compression bandaging and compression wrapping techniques. Measure and monitor limb volume and edema progression.
Educate patients and caregivers on:
Skin care and infection prevention Compression garment use Self-manual lymphatic drainage techniques Therapeutic exercises Long-term lymphedema self-management Recommend and assist with compression garments and lymphedema supplies. Monitor for complications including cellulitis, wounds, fibrosis, and skin integrity issues. Collaborate with physicians regarding treatment progress and recommendations. Document all lymphedema assessments and interventions in accordance with agency, Medicare, and payer guidelines. Maintain current knowledge of evidence-based lymphedema treatment practices. QualificationsRequired Active Florida Physical Therapist license. Current Certified Lymphedema Therapist (CLT) certification from an accredited lymphedema training program (such as ACOLS, Klose Training, Norton School, or equivalent). Minimum of two (2) years of Physical Therapy experience. Home Health experience preferred. Knowledge of Medicare Conditions of Participation and AHCA regulations. Current CPR/BLS certification. Current driver's license, reliable transportation, and auto insurance. Ability to travel throughout assigned service areas. Excellent communication and organizational skills. Experience with electronic medical records (EMR). Preferred Home Health OASIS documentation experience. Wound care experience. Oncology rehabilitation experience. Experience treating post-mastectomy patients. Bilingual (English/Spanish) preferred. Knowledge & Skills Strong understanding of Complete Decongestive Therapy (CDT). Proficiency in Manual Lymphatic Drainage (MLD). Compression bandaging and garment fitting. Knowledge of edema management and wound prevention. Excellent clinical assessment and critical thinking skills. Strong patient education abilities. Ability to work independently in a home health setting. Effective interdisciplinary communication. Knowledge of Medicare documentation requirements. Physical Requirements Ability to lift up to 50 pounds. Frequent standing, walking, bending, kneeling, and reaching. Ability to perform manual therapy techniques for extended periods. Ability to travel between patient homes throughout assigned territory.
