Responsibilities:

Assess referred patients that require rehabilitation services and develop personalized treatment plans Provide direct patient care, including evaluations, treatments, and education Establish and administer treatment programs with specific goals determined according to patients' capacity and tolerance Document and record patients' conditions, progress, and treatment plans Collaborate with other disciplines to provide patient-centered care Meet all regulatory requirements, including evaluations, weekly progress notes, recertification, and discharge documentations Perform all other duties assigned by the

Director of Rehabilitation Requirements/Qualifications:
