Job Description
We are seeking a dedicated and compassionate Physical Therapist (PT) to provide high-quality, patient-centered care in the comfort of patients' homes. What You'll Do Perform comprehensive physical therapy evaluations in patients' homes. Develop individualized treatment plans based on physician orders and patient goals. Provide skilled therapy services to improve strength, balance, mobility, endurance, and functional independence. Educate patients and caregivers on home exercise programs, safety techniques, and fall prevention. Collaborate with physicians, nurses, and the interdisciplinary team to coordinate patient care. Complete timely and accurate documentation in accordance with Medicare, state, and agency requirements. Participate in care conferences and communicate changes in patient condition. Supervise Physical Therapist Assistants (PTAs) as appropriate. Qualifications Current Florida Physical Therapist (PT) License. Graduate of an accredited Physical Therapy program. Current CPR/BLS Certification. Home Health experience preferred but not required. OASIS experience preferred. Excellent communication and organizational skills. Valid Florida driver's license, reliable transportation, and proof of auto insurance. Why Join Our Team?
